Information sheet for families describing the CA background check process for any adult working in a child care center.

2. Child Care Center Notification of Parents’ Rights

Explanation of Parents’ Rights regarding the center and licensing while child is enrolled in the center. Review and sign acknowledgement.

Authorization for emergency medical treatment should the child get injured while in child care.

Additional enrollment information required by California state licensing.

This form is required for any child who should not be served particular foods due to cultural/religious/vegan or vegetarian reasons, but excluding medical causes (i.e. allergies) or personal preferences (i.e. dislike of certain foods).

To be reviewed and completed by parent. Includes authorization and consent for emergency situations, persons the child may be released to; walk permission, photography and video permission, and acknowledgement of the Bright Horizons Family Guide or client equivalent.

To be completed by parent/guardian and child’s medical provider (when necessary), authorizing the administration of medication to child while at the center.

Record of the child’s physical and immunizations to be completed and signed by the child’s health care provider (top portion to be completed by parent/guardian)

16. Rights of the Licensing Agency

To be reviewed and signed by the child’s parent/guardian. Description of the rights of the licensing agency as it relates to your child’s records and interviewing children and staff.

17. Sunscreen and Insect Repellant Permission Slip Form

To be signed by family; valid for one year dependent upon state licensing regulations.

18. Topical Applications Administration Permission

Required form to be completed if you would like any type of over the counter lotion, ointment, or cream to be applied to your child.

To be completed by the child’s medical provider for a child with medically diagnosed allergies, detailing the child's allergies, symptoms, and treatments necessary in case of a reaction. Any medication(s) prescribed must be provided to the center.

To be completed by the child’s medical provider for a child diagnosed with asthma, detailing the child's triggers, symptoms, and treatment necessary. Any medication(s) prescribed must be provided to the center.
